Important Provider Information


 

All network providers accept new patients at the locations listed on this site.  However, information about the providers is updated on a daily basis. As a result, the information about the providers may have changed.  Please contact the provider office or contact customer service to confirm their participation, as their status will have an impact on your benefits.  When you see a provider that has left our network or if you see a provider or location not listed in this directory,  your out of network benefit will apply and your cost share will increase. 


How we build our vision network

Our vision network consists of a variety of provider types across the country: from large, national brands to small, independent practitioners. To ensure all members have access to providers who meet their needs, we look at the number and distribution of network access points in specific areas. We make outreach to providers as needed in order to recruit them to our network. We also accept requests from employers, members, and providers to accommodate needs and preferences.

 

If you do not see your local eye care provider as part of our provider network search results and would like to suggest that they become part of your Vision plan network, please utilize our provider nomination form.
 

Network Provider Quality

Providers must complete our credentialing verification process before joining the network and every three years thereafter. Between credentialing verifications, we monitor provider quality through a variety of efforts including our Quality Management Program. These efforts include monitoring:

  • Access to care
  • Quality of care and quality of service
  • Trends in grievances and appeals

California

Health Services from Non-Network Providers Paid as Network Benefits

If specific Covered Health Services are not available from a Network provider, you may be eligible for Network Benefits when Covered Health Services are received from non-Network providers. In this situation, your Primary Physician or other Network Physician will notify us and, if we confirm that care is not available from a Network provider, we will work with you and your Primary Physician or other Network Physician to coordinate care through a non-Network provider. If care is authorized from a non-Network provider because it is not available from a Network provider, you will be responsible for paying only the in-Network cost sharing for the service.

 
Timely Access to Care

Non-Urgent Appointment (ancillary provider) 15 business days

American Disabilities Act Notice: In accordance with the requirements of the federal Americans with Disabilities Act of 1990 and Section 504 of the Rehabilitation Act of 1973 ("ADA"), UnitedHealthcare Insurance Company provides full and equal access to covered services and does not discriminate against qualified individuals with disabilities on the basis of disability in its services, programs, or activities.  Interpreter services will be coordinated with scheduled appointments for Covered Health Services in a manner that ensures the provision of interpreter services at the time of the appointment.

Connecticut

Any provider included in this directory is listed for outpatient services.  In addition, the directory includes information regarding whether the provider is currently accepting new patients.

 

Colorado

Please note - UnitedHealthcare Vision has providers in every Colorado county except Baca, Bent, Cheyenne, Clear Creek, Conejos, Costilla, Crowley, Custer, Dolores, Gilpin, Grand, Hinsdale, Jackson, Kiowa, Kit Carson, Lake, Lincoln, Mineral, Moffat, Ouray, Park, Phillips, Pitkin, Rio Blanco, Rio Grande, Routt, Saguache, San Juan, San Miguel,  Sedgwick, Teller, Washington and Yuma.

Delaware

You have the right not to be balanced billed by a non-network provider who does not provide you a copy of the signed out-of-network disclosure in a timely manner as required by the state of Delaware.

If a provider’s entry contains a termination date, it means that the provider’s agreement with UnitedHealthcare ends on this date, and the provider is scheduled to leave UnitedHealthcare’s provider network. However, UnitedHealthcare and the provider may negotiate to reach a new agreement.  If UnitedHealthcare and the provider reach a new agreement, the provider will remain in-network and the term date will be removed from the directory.

Members should call the Vision Customer Service Center to speak to a representative to get help locating a provider, or for help with questions related to finding a provider.  Members can also ask staff to assist them in making appointments with providers


Nevada

Ohio

When a provider no longer participates in the network, or has changed participation status, additional liability is likely to result. Soon after the provider changes their status, out of network penalties or increased cost sharing will result. In some cases, extension of in network or greater network benefits may be available.

The providers available through this directory may not necessarily reflect the full extent of UnitedHealthcare's network of contracted providers. There may be providers or certain specialties that are not included in this directory that are part of our network. If you don't find the provider you are searching for, you may contact the provider directly to verify participation status with UnitedHealthcare's network, or contact Customer Care at the toll-free number shown on your UnitedHealthcare member ID card. We also recommend that, prior to seeing any physician, including any specialists, you call the physician's office to verify their participation status and availability.


 

Our Networks of Physicians and Hospitals

All providers that participate in one or more of UnitedHealthcare Vision networks meet our credentialing requirements. These requirements comply with National Committee for Quality Assurance credentialing standards. All of our networks are evaluated regularly against our national availability standards. This process ensures we have enough optometrists and ophthalmologists in the right locations for our members.
